I, [model’s name], hereby grant Cat’s Imaging & Design the right to use my name, likeness, voice, and image (collectively, the “Materials”) in connection with any advertising, promotion, or marketing materials created by Cat’s Imaging & Design.

I acknowledge that I am not entitled to any compensation for the use of the Materials by Cat’s Imaging & Design. I further acknowledge that Cat’s Imaging & Design is the exclusive owner of all rights, title, and interest in and to the Materials.

I hereby release and discharge Cat’s Imaging & Design and its officers, employees, and agents from any and all claims, demands, and causes of action that I may have or may hereafter have by reason of anything contained in the Materials or by reason of the use of my name, likeness, voice, or image as authorized by this Agreement.

I represent and warrant that I have the right to enter into this Agreement and that the use of the Materials by Cat’s Imaging & Design as authorized herein will not violate any rights of any third party.

This Agreement contains the entire understanding between Cat’s Imaging & Design and me and supersedes all prior agreements and understandings, whether written or oral, relating to the subject matter of this Agreement.

This Agreement shall be binding upon and inure to the benefit of Cat’s Imaging & Design and me and our respective heirs, executors, administrators, successors, and assigns.

I have read this Agreement and fully understand its terms. I execute it voluntarily and with full knowledge of its significance.
